We will continue to evolve and make changes to the program as described in the About Us based on the needs of the community and the demand that exists. We will provide additional details of the evaluation/try-out process post registration.

Medical Releases and Certification
- We do not collect paper Medical Releases. Please do NOT mail anything in to our PO Box or bring a Medical Release to evaluations. Once your child is assigned to a team, your coach will ask for a guardians signature on the team Medical Authorization & Liability Release that they will carry with them in the gyms. Coaches will be given players' emergency contact information electronically via our database.
- Player certification documents are not being collected for Division players (new or returning). With the completion of online registration, guardians confirm player age, grade and address are all accurate.
Foothill Youth Basketball reserves the right to request any document for verification purposes. Any players who knowingly falsify their information to gain entrance into our program may be denied participation. Player certification documents may be required prior to participation in Post Season tryouts. Detailed information on these certification requirements will be sent to appropriate participants.
